Two problems dominate here. The first is commercial kitchens: an optical smoke detector over a service pass will go off during every busy service, so it gets covered, and a covered detector is worse than no detector. Heat detectors, correctly sited, plus a proper interface to the extract and the gas shut-off, is what makes a restaurant system survivable. The second is the buildings themselves. In a conservation area or a listed terrace, cable routes have to be planned with the constraints in mind, because surface trunking down a period staircase is not going to be accepted and improvising on the day wastes everyone's time.
Under the Regulatory Reform (Fire Safety) Order 2005, the Responsible Person for the building — usually the employer, owner, landlord or managing agent — has to make sure a fire risk assessment is carried out and the fire precautions are kept working. Getting the design wrong is not just a technical problem, it is that duty left unmet.
